Prologue

MS changes you. It makes you more aware of your past and present faults. Even of your future ones if you’re not careful. It makes you scared, anxious and stressed. It pumps fear, uncertainty and aggitation. And all of a sudden something changes. You change. For the better? For the worse? Only time will tell. Wait and see. Have patience and read through the lines. The Metamorphosis has begun!

Friday - 07:00 a.m.

For Heaven’s sake! Does this alarm has to ring e-very morning? OK, OK! I’m up, I’m up! There! I turn you off, stupid alarm!! (like you turned me off a few seconds ago). Thank God it’s not raining again. The good thing is that today is Friiiidaaay! Weekend at last. What should I wear today? … A pair of jeans, this shirt… and I think I’ll take the black blazer too. The rain could make us all a big surprise again. The coffee smells good today! Hm, a ciggarette would be nice! Five minutes and it’s all done!

Ooooukey! Let’s put some make-up on! Why does my eye socket hurt? It must have been the damn air conditioning at the office. And I told Miranda not to overuse it! It blew right in my face. Oh, well! It will pass… For two days I’ll be out of the office and cold-air free! … Where did I put my mascara…? Here it is! … Ahhhh! Damn it! Can’t believe I hit my eye! How sleepy can I be? Ahhh, it stiiings! Let’s wash it off… no, I can’t do that! I’ll be late… Hmm… A Q tip will do the trick! It always does! … Yay! I got rid of the sting. It might have been a lash. I don’t have time to worry about it. It’s already 8 o’clock! Another 10 more minutes and I’ll be out the door.


08:45 a.m. - At the office

I’m the first one to get here! Again… Opeeen the laapptoop and … go smoking again… it’s only 08:50, I have a little time left.. and it’s Friday after all… I wonder if someone will reach the sales target this month… Haha! Wait and see. Who knows what could happen! It would be so nice if my eye would stop itching.. and that pain! I’ll stop at he pharmacy on my way home.. they must have something to help me deal with this! … Somebody came in… 
- Hi, Miranda! I was waiting for you! What’s up?
- Oh, I just came from dropping my little one at the kindergarden. God, he made such a fuss out of being left alone. Again. By me!
- Poor thing, Miranda! I thought he was already getting used to his new schedule and all…
- It seems like he needs a little more training or.. dunno! Maybe I’ll spare him of kindergarden and hire a nanny ‘till he’s able to go to school. Haha!... Is coffee ready?
- Yes, I made a fresh batch. Help yourself. We have milk in the fridge.
- Finally! Oh, by the way! We’re alone at the office today. Between home-offices and vacations we’re left to concentrate on our sales strategies.
- Yeah, right! As if that was the reason for the others being at home. Oh, well! Let’s get on with the day. I finished my cigg, so I’ll be waiting for you in the office. You’re OK with that?
- Yes, no problem. Go, I would be there shortly.


11:17 a.m.

My eye still hurts! Did I got something in it as I was walking here from the bus station… I don’t think so… Miranda?!
- Yes..
- Could you please help me with something?
- Sure, what’s the matter?
- I think I got something in my eye. Could you take a look and tell me if you see something?
- … Hmm… there seems to be no .. wait! I see a little .. I dunno what it is.. a thin puff, let me get it out for you.

13:30 p.m.

My eyes are tired.

- Miranda?! I’m taking a short brake. I’ll be in the kitchen if you need me, OK?
- OK, dear! Go relax for a bit. You haven’t got off that chair in three hours!

I can’t smoke! It’s too hot outside and having this window open feels like opening the gates to Hell! Hehe! That’s why I love summer! NOT! Neah! Don’t feel like smoking anymore. Let’s get back to work. A few more phone calls and a bunch of emails and I’m home free! Weekend at last!

- Back already?
- Yep! Too hot outside!
- Are you done with calls today?
- No, I have a few more left, but I’ll wrap it up in an hour or so.
- Good!
- But my eye still stings!
- Meeeh, you just have the remaining sensation. I got that thing out, remember?
- Yeah, but I can almost feel it still in there. And my eye is tired. Almost blurry.
- It will pass. Stay calm and take short brakes. You tire your eyes looking constantly at that screen.
- I’ll relax them a bit!


17:55 p.m.

I can almost feel a slight headache rising through…

- Miranda?! (damn, my socket feels like it's burning!) Miranda… I’m done for the day! How’s your work going?
- Uhhm, not so good! I think I need to work on it for about an hour more. Are you OK?
- I hope so! My head hurts a bit and that damn eye still bugs me!
- Oh, poor you! Calm down, it will be ok! Go buy some eyedrops and some ibuprofen and you’ll be fine!
- Yeah, that was just my plan! Thanks for the advice. See you on Monday! (If my eye gets better!)

Wait, my eye cleared a bit! Yay! Let get those drops and get home.

- Hi love! Didn’t expect too see you home so early!
- Hey gorgeous! … We finished the viewing and there was nothing more left to do so he sent us home!

Oh, well… it’s Sunday! Bummer! Tomorrow that alarm is going to ruin my sleep again! But in the meantime, let’s enjoy the beautiful July weather outside!

- Thanks! Breakfast was sooo good! I’m living with a real top chef! Haha!

I’m so tired! And this Sun is hitting me right between the eyes. Let’s move a bit. Ouch! I got some ciggarette smoke in my eye. Wait! It’s not the smoke! The pain came up agin. I’m feeling a little headache comming too. Ehh… get over it.. I’m tired… I’ll sleep it of!

- Could you read this message for me? I can’t do it untill I finish rolling this chicken, or we’ll have burned barbecue! Heheh!

Monday - 10:00 a.m.

Why am I so anxious? It’s not my first eye control. That eye sore, or viral conjunctivitis, or what the Hell it was passed so quickly… I bet this one will be the same!

- Good morning Alex! What brings you here? I hoped our meetings would be fewer.
- Oh, just a check-up. My right eye has been hurting me for the whole weekend. I think I also got a headache from it a few times.
- Take a seat here, please. Let us check it! … Allllriiight! Can you read this for me, please? …. OK! Good! Now this line, please! ….. This one… OK! You did good!
- I’m usually good at this! Hehe!
- Yes you are! Put your head here, please! Keep your eye still… don’t move...or blink.. OK, let’s do the other one!

- Same ones, yes! … Did you have any reading problems.. as not being able to distinguish between letters or needing to squint to see a word..
- No, I did not have issues like that! Why?
- Because from what I can see here, there’s nothing wrong with your vision. Did the pain upset you much?
- Yes. It was constant, and it hurt as I rotated or moved my eye from side to side.
- Let me see those eyes again! Put your head here… OK… and now the other eye...
OK! As I said before, your vision is perfect, but I think there’s something wrong with the optic nerve. Did you ever took a visual field exam?
- No.
- I would want you to have your visual field checked up and go to a neurologist to see if all things are alright. Then comeback and we’ll look at the results.
- Why are these necessary?
- As I said, your vision is good, and I can’t see what is wrong. A visual field chart and a neurologist’s opinion would help me make the right choices for you. Here is your letter of recommandation… your diagnosis is retrobulbar optic neuritis. Make an appointment for these two, and then come back here to look at the data together.

Diagnostic: SM. Ce sa discuti cu doctorul?

Primirea unui diagnostic de scleroza multipla te poate face sa-ti fie frica, sa te simti depasit de situatie. E important sa iti aduci aminte ca nu esti un caz izolat. Se estimeaza ca in intreaga lume exista aproximativ 2.5 milioane de oameni care traiesc cu scleroza multipla.
In Romania, numarul acestora este de aproximativ 10.000 de persoane, majoritatea destul de tinere.





Este normal sa ai o multime de intrebari in legatura cu noul diagnostic. Aflarea acestor raspunsuri si informarea in legatura cu afectiunea ta, te pot ajuta sa te simti mai bine. Mai jos ai trei subiecte esentiale despre care sa discuti cu doctorul tau urmatoarea data cand va vedeti.



Ce simptome voi avea?


Este posibil ca tocmai acestea sa fi contribuit la punerea diagnosticului de la bun inceput. Fiecare persoana experimenteaza simptome diferite, asa ca este destul de dificil sa ti se ofere un prognostic privind modul in care va evolua afectiunea sau exact ce simptome vei simti. Simptomatologia ta depinde de locatia fibrelor nervoase afectate, de unde anume se gasesc leziunile (inflamatia) la nivelul creierului.


Manifestari comune ale SM includ:


  • amorteli si/sau slabiciuni ale corpului;
  • miscari oculare dureroase;
  • pierderea sau perturbarea vederii, de regula doar la un ochi;
  • oboseala fizica (extenuare fizica si/sau psihica);
  • furnicaturi sau senzatie de “piscaturi”;
  • durere (sunt afectati nervii responsabili de durere);
  • senzatii de curentare (de obicei atunci cand misti gatul in fata);
  • tremuraturi;
  • dificultati de echilibru;
  • ameteli sau vertij;
  • dificultati de tranzit intestinal sau de urinare;
  • vorbire neclara, “impleticita”;


Cu toate ca parcursul exact al afectiunii tale nu poate fi prevazut, aproximativ 85% din cei diagnosticati cu SM au forma recurent-remisiva (SMRR). SMRR este caracterizata printr-o exacerbare (puseu / episod / recurenta) a simptomelor pe care le ai deja sau aparitia altora noi, urmata de o perioada de remisie (de inactivitate), care poate dura luni de zile sau chiar ani.



Ce optiuni de tratament am?


La momentul de fata SM este incurabila, dar exista o multitudine de tratamente eficiente disponibile. Tratamentul are trei scopuri principale:


  • modificarea cursului afectiunii, prin incetinirea activitatii SM si pastrarea unei perioade de remisie indelungate;
  • tratarea exacerbarilor (puseelor);
  • managerierea simptomelor;


Terapiile imunomodulatoare (T.I.)
Reduc in mod eficient numarul de recurente (pusee) si incetinesc progresia afectiunii. Exista:


  • tratamente administrate intravenos, la una din clinicile specializate,
  • tratamente injectabile (subcutanat si intramulcular) administrate acasa
  • tratamente orale, de asemenea administrate acasa.


La momentul de fata, in Romania exista doar tratamente intravenoase si injectabile, urmand ca tratamentele orale sa fie introduse probabil candva in viitor.


Tratamentele recurentelor (puseelor) (T.P.)
Unele pusee se rezolva fara tratament. Atunci cand sunt severe, doctorul poate prescrie un tratament corticosteroid, cum ar fi Solumedrolul, cu scopul de a reduce inflamatia cat mai rapid. Administrarea se face intravenos, in perfuzii zilnice, administrate intre 3 si 5 zile, in functie de gravitatea puseului.


Tratamentele simptomatice (ale simptomelor) (T.S.)
Acestea depind de simptomele avute (ex: durere, rigiditate, spasticitate, etc). Aici sunt incluse si tratamentele afectiunilor ce pot insoti SM, cum ar fi anxietatea, depresia si problemele urinare, si asa mai departe.


Pe langa acestea, ti se mai pot recomanda si terapii de reabilitare (fizioterapie), kinetoterapie, etc, in functie de nevoi.



Ce mai pot face pentru a-mi manageria SM-ul?


Alegerile de viata sanatoase sunt un element de baza in managerierea simptomelor SM.


Vei beneficia din plin de rezultatele pozitive ale miscarii:


  • imbunatatirea fortei fizice;
  • cresterea rezistentei;
  • imbunatatirea functiei renale

Este obtinut din struguri si plus de asta, contine antioxidanti. Bineninteles, si acesta consumat cu moderatie.


Desi nimeni nu poate prezice cursul afectiunii de-a lungul vremii, SM poate fi controlata prin tratament adecvat si printr-un stil de viata sanatos. Un stil de viata SMart. SMart Choice!

Ai SM? Ai incredere in tine!

Hei, tu! Da, tu! Pana nu demult te simteai invincibil, puteai face orice, oricat, oriunde. Grijile lipseau, singurul stres major erau ziua de luni si probabil traficul sau un proiect cu un deadline mai strans.





Cum incepe totul?

Primele simptome. O amorteala a mainii, o pata de durata pe unul din ochi si poate migrene. Te-ai ingrijorat. Ai ajuns la medic. Ai facut poate primul RMN din viata ta, ti-au fost pusi electrozi pe cap, ai aflat ce presupune un control neurologic. Ai fost intepat cu multe ace, dintre care unul poate chiar intre vertebre, pentru o punctie lombara. Ai simtit sentimentul acela de neliniste si dorinta de a pleca mai repede acasa, pentru ca “Oricum n-am nimic! E de la oboseala! De la tigarile alea nenorocite!”. Si dintr-o data lumea ta s-a schimbat. Neurologul te-a anuntat ca ai scleroza multipla, ca e o afectiune incurabila, ca se poate tine sub control si… ca va trebui sa traiesti cu ea de acum incolo.

In mintea ta s-a produs un vid. Esti confuz. Ai refuzat sa crezi ceea ce auzeai. Ai cautat dovezi care sa nege diagnosticul. Ai plans, ai urlat, ai plans inca putin. Ai refuzat sa accepti. Nu tu, nu acum! Vrei sa mai traiesti putin. Vrei sa fii liber. Simti cum peretii se strang in jurul tau si vrei sa evadezi. Dar pericolul e in tine. Asa ti s-a spus. Ca e o boala autoimuna, degenerativa, care duce la dizabilitate. Si ti-e frica. Si plangi. Te enervezi. Ai momente in care te impaci cu gandul, doar pentru a rabufni cu si mai multi nervi. Si te simti singur. Si vrei sa crezi ca ai visat.

Ai doua variante: accepti ceea ce ti se intampla si mergi mai departe, adaptandu-te sau te cufunzi intr-o viata traita mecanic, in frica si negare. Sau poti alege un amestec: sa mergi mai departe insa cu frica. Dai nastere anxietatii. De ceva timp te injectezi. E doza necesara pentru a tine SM-ul sub control. Altceva nu stii ce sa faci.




Trage aer in piept si citeste mai departe

Te simti la pamant si crezi ca nu mai poti merge mai departe? Ai tot dreptul sa te enervezi si sa-ti fie frica. Ai tot dreptul sa strigi cat te tin plamanii “DE CE EU? DE CE ACUM?”. Mi-as pune multe semne de intrebare daca nu ai face asta.

Trebuie sa intelegi ceea ce ti se intampla, sa te scuturi de frica, furie si tristete si sa mergi mai departe. Trebuie doar sa crezi si sa actionezi in acest sens. Putin cate putin. Zi de zi. E un nou inceput. Ai ocazia sa faci lucrurile bine, e ca si cum ai incepe de la zero. Daca nu schimbi ceva, totul va ramane la fel. Cea mai importanta schimbare pe care o poti face e la modul cum simti ceea ce ti se intampla.

Starea de spirit infuenteaza imens evolutia SM-ului. Si stii de ce? Pentru ca te blocheaza in emotii negative si stres. Pentru ca simti cum iti anihileaza pofta de viata si cheful de a mai face ceva cu viata ta. Crezi ca totul s-a terminat. Adevarul e ca abia acum incepe. In special daca ai fost diagnosticat pana in 30 de ani. Acesta e punctul de plecare in viata. Pana acum a fost doar antrenament. Ai curaj si traieste-ti viata asa cum visai pana nu demult. Adapteaza-te si cauta in permanenta motivatii pentru a evolua. Cel mai important motiv esti chiar tu. TU si dorinta ta de a trai, de a te misca, de a simti viata prin toti porii si cu toate simturile.

E o lupta inegala, dar crede-ma ca impreuna putem pune SM-ul la pamant! Da-l cu capul de pamant! N-are niciun drept asupra ta! Lupta pentru dreptul tau la o viata linistita si normala.
SM-ul iti consuma resursele, dar tu poti merge mai departe. Indrazneste sa traiesti si sa iti iei viata in propriile maini. Esti invincibil, chiar si SM-ul stie asta. Nu te lasa batut si tine capul sus. Viata oricum e doar o plimbare in montagne-rousse. Nimeni n-a spus ca a trai va fi usor, dar cert este ca merita incercat. Da-ti o sansa. Dovedeste-le celor care te-au subapreciat si stigmatizat ca esti mai mult decat credeau ei. Esti mai puternic decat crezi.




Ai incredere in tine! Preia controlul!

  • Informeaza-te! Cunoaste-ti… “inamicul”! Pentru mai multe informatii, citeste!

  • Contacteaza clinica de neurologie care se ocupa de programul national de tratament in zona ta.
  • Depune dosarul pentru tratamentul potrivit pe care sa ti-l administrezi cu constinciozitate. 
  • Intretine o relatie de parteneriat cu medicul tau neurolog. Sansele sunt sa va vedeti destul de des in anii ce vor urma. 
  • Noteaza intrebari, cere-i parerea, dar nu-l agasa si nu crede ca 8-9 luni de informare bat zeci de ani de practica. Dar fii liber sa alegi cel mai bun doctor sau pe cel pe care il simti aproape de tine.
  • Fii activ (socializeaza, citeste, razi, descopera locuri noi).
  • Fa miscare (yoga, aerobic, alearga, inoata… orice poti face si faci cu placere).
  • Renunta pe cat posibil la alimente procesate, la toxine (ex.: Cola, tigari, fast food)
  • Nu te mai stresa pentru toate nimicurile!
